Use the photograph page in the Class D package
FDACS form 16007 includes a dedicated photo specification. It asks for everyday clothing, a neutral expression and both eyes open. Uniform-like clothing and camouflage are excluded except as described for daily religious attire.
Do not import another agency’s glasses rule
The FDACS sheet allows normally worn prescription glasses when there is no glare. Its rules also address headwear and medical needs. Use this application’s instructions rather than automatically borrowing the US passport glasses rule.
Check the photo separately from the application paperwork
Merodim tip: keep the photo in a protective sleeve while assembling the package. Before sending, inspect the surface for marks and confirm you selected the Class D instructions rather than a different licence class.
Problems and practical fixes.

- A work-uniform portrait is used
- Take a fresh photo in suitable everyday clothing rather than digitally replacing the clothing.
- The download is printed on ordinary office paper
- Use a photo-print service or photo paper suitable for the required finish.
- The picture is stretched to fill the square
- Use a crop from the original. Stretching changes the proportions of the face.
