Find the instructions for your form

There is no single photo format for every federal application. An agency may ask for a photograph as supporting evidence, collect one at an appointment or use an image already on file for the finished document.

Identify the agency and form number first. Then check whether the photograph is an attachment you must supply or one the agency will take. This distinction matters even when both instructions use the words “passport-style photo.”

DS-160 visa applications include a photo upload

The Department of State publishes digital image specifications for visa applicants, including file type, dimensions and file size. Check the embassy or consulate’s appointment instructions as well as the upload requirements.

If the DS-160 confirmation page shows an X where the photo should appear, the State Department’s FAQ says the upload failed. It directs applicants to provide a printed photograph with the confirmation page and contact the embassy or consulate for instructions. A successful upload still does not establish that every photo requirement has been met.

USCIS collects photos for identity documents

USCIS’s December 12, 2025 policy states that it will not use or reuse self-submitted photographs for identity documents. It sets conditions for reusing photographs collected during biometrics services and requires new photos for specified applications.

Read this alongside your current form instructions and appointment notice. A photo requested as application evidence and the photo used on the final identity document can serve different purposes. Do not skip requested evidence or a biometrics appointment because you already have a prepared portrait.

Resolve conflicting instructions before paying for a photo

If a form asks for an attachment but a general page describes appointment capture, ask the agency which photograph each instruction refers to. Include the form number, application method and the wording you are trying to clarify.
